How it works

Fill out and submit a referral form.
This is a place to start when you need help, and you don’t have to figure things out on your own.

Get paired with someone who understands.
You will be matched with a trusted care coordination organization partner and a community health worker or promotor/a de salud who may share your culture, language, or lived experience.

Have support that stays with you.
Your community health worker or promotor/a de salud will take the time to listen and work with you to find the right support available. They will check in to see how things are going over time, in a way that respects your values, your culture, and your needs.
North Sound Community Hub serves as a central connection point, pairing you to a community health worker or promotor/a de salud. They will help reduce difficulties you face accessing services and minimize the number of times you tell your story to doctors and providers. As your partner, they will help make sure you receive care that’s timely and personalized to your needs.

North Sound Community Hub works with local care coordination organizations to tackle gaps in health care, as well as barriers that keep people from getting the care they need. Through this coordinated approach, we’re able to track health outcomes — for individuals, care teams, and our region as a whole — so that all the systems that keep people healthy in our region continue to improve and work better for everyone.
