North Sound ACH supports a regional care coordination hub, which takes referrals from across the region about individuals and families struggling due to physical, behavioral, and social needs. The Community Hub serves as a central registry for a network of care coordination agencies (CCAs), helping to eliminate structural barriers while reducing the duplication of services. The Hub also serves as the central infrastructure for tracking and measuring specific health outcomes, agency outcomes, staff outcomes, and opportunities for coordinated quality improvement efforts.


CCAs are organizations contracted with the Community Hub to deliver direct care coordination services to community members. The Hub and its network of CCAs work in partnership to identify high-risk individuals and direct them to appropriate service providers.
How does it work?
As an ACH (Accountable Community of Health), we are part of a statewide coordinated effort to provide regional infrastructure for sustainable financing for community-based care coordination. See our video (click here), produced by consultant firm DH.
Each Community Care Hub of Washington connects and coordinates a regional network of social and health services providers, so people can get the care they need when they need it. Hub providers and partners reflect their client’s culture and respect them as individuals with a unique set of needs.
People are referred to their regional hub in different ways:
- – Through a referral from a health care or social services provider.
- – Or by completing a self-referral form.
Once engaged, people who need social and health services are matched to a community-based worker, based on culture and language preferences, who will walk them through the process of connecting to services from start to finish, following up to ensure they received the care and support they needed.
