Project Manager – Community HUB

Salary Range: $62,308 – $81,000 (depending on qualifications)

This Project Manager position will be part of a team that coordinates, trains and supports care coordination across the North Sound region, overseeing assessment, analysis, and performance of a care coordination HUB and its contracted partner agencies. The HUB utilizes clinical and community care coordinators, with an intentional emphasis on supporting community health workers in key roles. Duties include planning, stakeholder engagement, use of cloud-based collaboration tools, monitoring data integrity, and preparing data reports. Regular travel among the five-county service area is required, including occasional evening and weekend meetings.

The successful candidate will have knowledge of the social determinants of health, health delivery systems, and the growing community health worker workforce. A relevant 4-year degree or equivalent work experience with community health workers is required. North Sound is especially looking for candidates who bring diverse experience and background to our team – candidates of color, people with disabilities, veterans, homelessness, LGBTQI+ as examples but certainly not exclusive.
