Director of Population Health: This position will be responsible for promoting, advising, and directing a cohesive strategy across the whole person care continuum. The Director will oversee the initiatives and strategies to identify and address care gaps within the population, especially those with chronic illness and diseases, and taking into consideration the social determinants of health in action planning. The Director of Population Health will provide direct supervision and mentorship to assigned members of the North Sound ACH team, and will engage and promote a welcoming and open communication environment.
This is a cooperative and dynamic position, where the successful candidate will collaborate with staff and the leadership team to create action plans, develop and design programming and will be part of the decision-making process to support the purpose of the North Sound ACH.
