States and the federal government are investing heavily in initiatives to transform the health care delivery system, with states committing substantial resources to projects aimed at achieving the Triple Aim goals of better population health, better health care, and lower costs. However, these state investments may fail to achieve their goals without a plan to address the persistent disparities in health status and outcomes that plague underserved communities. Without addressing the disproportionately poor health outcomes of certain races or zip codes, states will continue to struggle to reduce costs, improve health, and improve quality.
