Organizational request form for COVID-19 assistance — organizational financial assistance, vaccine clinics, testing supplies, personal protective equipment, and COVID care kits. All requests are welcome but we will prioritize those who are working with underserved communities, providing community-based care coordination and utilizing essential workers like CHWs and promotoras.
