The Community HUB serves as a central registry of referred individuals for a network of care coordination agencies (CCAs), helping to eliminate organizational silos and reduce the duplication of services. The HUB serves as the central infrastructure for continuous tracking and measurement of specific health outcomes, agency outcomes, staff outcomes, and opportunities for coordinated quality improvement efforts.
The population of focus for the North Sound Community HUB are individuals with co-occurring behavioral and physical health conditions, who are also experiencing confounding social needs. Specifically, our pilot population is individuals experiencing behavioral health concerns with a compounding condition. Review full population criteria here.
